Breast implants involve placing a silicone or saline-filled device either under the breast tissue or under the chest muscle to increase size and improve shape. Implant options have advanced significantly and been refined over several decades, providing a custom approach to increasing the size of a patient’s breasts whether they desire a proportional breast enhancement or more dramatic, voluptuous breast contours.
In contrast, fat transfer augmentation takes fat from another part of the body — commonly the abdomen, thighs, or flanks — via liposuction, purifies the fat cells, and then injects them evenly along the breasts to soften contours, provide coverage for rippling, or mildly increase breast fullness. This method essentially uses the body’s own tissue to enhance breast volume, offering a more subtle and natural result while also contouring the donor area. That said, fat transfer cannot substantially increase breast size in the same way as implants.
Ideal Candidates for Breast Implants
Breast implants are usually most beneficial for women who want to meaningfully enlarge their breasts, who require breast reconstruction after cancer removal surgery, or who aim to enhance the breast shape and/or profile. The most successful outcomes are generally achieved for:
- Those seeking a notable size increase: Implants can enlarge one’s breast size in ways that fat transfer simply cannot match. If you’d like to enhance your breasts by two or more cup sizes, implants are typically the only realistic option since fat grafting is limited by how much fat can survive the transfer process, and how much volume the breast tissue can accommodate at once.
- Individuals with very little donor fat: Fat transfer requires a sufficient amount of harvestable fat elsewhere in the body. Naturally lean individuals, or those who have already had extensive liposuction, often don’t have enough donor tissue to achieve meaningful breast volume through grafting alone. For these candidates, implants remain the more practical choice.
- Patients wanting specific shape control: Implants come in a wide range of shapes (round or teardrop) and profiles (low, moderate, high, or ultra-high projection). This variety allows Dr. Goldberg to sculpt a very specific look, whether replenishing upper pole cleavage or providing a more natural teardrop slope. Fat transfer, by contrast, is more limited in its reshaping capabilities since it relies on the existing breast footprint.

It’s worth noting implants may require replacement or breast revision at some point, carry a potential risk of capsular contracture, and involve a more extensive recovery period than fat transfer — typically one to two weeks are required before returning to normal activities, with full recovery over several weeks.
Ideal Candidates for Fat Transfer
Fat transfer breast augmentation is commonly used in complement with implants, or during breast revision cases in which the implant edges are seen or felt through the skin. As a standalone procedure, fat grafting to the breasts can benefit:
- Those seeking a natural, subtle enhancement: Since fat transfer uses a patient’s own tissue, the result tends to feel and move like natural breast tissue rather than a device. Patients who want a modest increase — usually up to one cup size — while preserving a soft, natural silhouette can be good candidates.
- Patients wanting to avoid implant-related risks: Fat transfer eliminates concerns specific to implants, such as capsular contracture, implant rupture, rippling, or the eventual need for device replacement. For patients wary of having a foreign object in their body, or who have had negative experiences with implants in the past, fat transfer may be an appealing alternative.
- Individuals with mild to moderate volume loss: Patients who have experienced mild volume loss due to aging, breastfeeding, or modest weight fluctuation may find that fat grafting restores youthful fullness without the need for implants.
