The Ultimate Guide to Rhinoplasty in NYC: What to Expect Before, During, and After Surgery

Rhinoplasty, often known as a “nose job,” is one of the most sought-after aesthetic procedures in NYC. Whether you’re aiming to enhance facial harmony, improve breathing, or correct a previous surgery, New York City offers access to some of the world’s top plastic surgeons. As a Castle Connolly Top Doctor® and a board-certified plastic surgeon, Dr. Neal Goldberg is committed to guiding each patient considering rhinoplasty through everything they need to know about surgery — from their initial consultation to recovery and results.

Why Do NYC Patients Choose Rhinoplasty?

Rhinoplasty is a surgical procedure that reshapes the nose to enhance appearance or function. Common goals for men and women in NYC and beyond involve changing the size, shape, or angle of the nose, correcting bumps or depressions, refining the nasal tip, or addressing breathing issues caused by structural abnormalities (such as a deviated septum). Cosmetic rhinoplasty focuses on improving the nose’s appearance, while functional Rhinoplasty aims to correct breathing issues and restore proper function. Many patients in New York City undergo a combination of both for optimal results.

Before Surgery

With both female and male rhinoplasty, the process begins with a thorough consultation. At this appointment, Dr. Goldberg will:

Your personalized surgical plan will be developed to consider your unique anatomy, facial balance and harmony, and any functional concerns (e.g. breathing difficulties). Dr. Goldberg will also review potential risks, surgical techniques (open vs. closed rhinoplasty), and anesthesia options.

During Surgery

Rhinoplasty is performed under general anesthesia in an accredited surgical facility and typically takes between 1.5 to 3 hours, depending on the complexity of your case. Dr. Goldberg will choose between two main surgical approaches:

During the facial plastic surgery procedure, the surgeon may reshape the nasal bones, refine cartilage, remove excess tissue, or even graft cartilage (often from the septum, ear, or rib) to achieve the desired shape and function. If breathing issues are present, such as a deviated septum, those can also be corrected simultaneously. Once the structural changes are complete, the skin is carefully re-draped, and the incisions are closed. A nasal splint is typically applied to help maintain the new shape and support the nose during early healing.

After Surgery

Immediately after surgery, patients are taken to a recovery area where they’re monitored as they wake from general anesthesia. A nasal splint is placed on the outside of the nose; in some cases, internal splints or packing may be used to support the nasal structure and manage swelling. Bruising and swelling, particularly around the eyes, are common and usually peak within the first 48 to 72 hours. Most patients experience mild to moderate discomfort during rhinoplasty recovery, which can be managed with prescribed pain medications. Keeping your head elevated and applying cold compresses during the first few days can help reduce swelling.

In the first week, most patients take time off work or school to rest and allow initial healing. Sutures and splints are typically removed between five to seven days post-op. At this stage, while swelling begins to subside, the nose may still appear puffy or uneven — this is normal and temporary. You may be cleared to return to light daily activities, but any strenuous exercise or contact sports should be avoided for several weeks.

Over the next few months, swelling will gradually diminish, and the new shape of the nose will begin to refine. While early improvements can be seen within a few weeks, the final results of rhinoplasty often take up to 12 to 18 months, particularly when tip reshaping is involved. Routine follow-up appointments are scheduled to monitor progress, ensure proper healing, and address any concerns. With patience, care, and a skilled surgeon, most patients enjoy a beautifully refined nose that enhances facial balance and self-confidence.
